## Phase 0: Clinical Validation Gate

Deliverables: clinical validation checklist, CPI/Nexus addendum checklist, risk/compliance checklist, pilot charter template, drift-control rules.

Evidence gate: every clinical/risk checklist row has an owner, decision, and date.

Stop rule: if clinical validation cannot be scheduled inside 14 calendar days of sponsor approval, pause and escalate to executive sponsor.

## Phase 1: Pilot Build

Deliverables: governance packet, facilitator shell, quick card, scenario architecture spec, scenario log, survey, dashboard spec, review templates, printing pack.

Evidence gate: facilitator dry run passes with two clinical observers.

Stop rule: any scenario that fails the scenario architecture spec stays out of the deck.

## Phase 2: 90-Day Pilot

Deliverables: weekly review records, monthly review records, scenario logs, staff pulse results, family-loop/debrief samples, stop-criteria review.

Evidence gate: process, learning, outcome, and balancing measures have data source owners.

Stop rule: unsafe huddle contact, staff shaming, policy drift, threshold confusion, or missing metrics for two reporting cycles pauses the pilot.

## Phase 3: Scale Decision

Deliverables: filled 90-day scale packet, quality recommendation, executive decision.

Evidence gate: all four measure categories have threshold lines marked met, not met, or inconclusive.

Stop rule: if any stop criterion fired during the pilot, the recommendation is revise and re-pilot, not scale.

## Phase 4: Enterprise Rhythm

Deliverables: annual calendar, quarterly scenario refresh, monthly dashboard, quarterly executive report, annual program review.

Evidence gate: each quarterly refresh has clinical validation and version control.
